The three primary reasons are:

1. Your Network and connections

2. Your 'Pitch' and presentation

3. Lack of Relevance (or the perception)

 

Statistics tell us that your next role is 98% likely to come out of your network and connections.  Makes sense considering a first impression only takes 7 seconds.  Having a connection to the role stretches that opportunity out for a first impression. It also provides context and more receptive audience for your 'pitch' and relevance.
